I give my permission to Children's Hospital Colorado to broadcast live, make a video and/or audio tape, take photographs, place presentations and/or handout(s) on Children’s Colorado website and/or CD and/or flash drive, generate computer images or other images of me.

I understand that I will not be paid for the broadcast or making of this tape or images and have no rights to the final product or raw materials.  

I release Children's Colorado, its officers, directors, employees and agents, from any and all claims whatsoever of harm or otherwise which may occur from the making, showing, or distribution of this broadcast, video and/or audio tape, photographs, computer images or other images. 

By participating in Children's Hospital Colorado video broadcast service, I am confirming that I have secured all of the necessary clearances, approvals and consents for the materials used in my lecture.
