The Dartmouth NIPD Program: clinic, science, and a treatment approach to PANS/PANDAS

Learning Objectives

At the conclusion of this activity, learners should be able to:

• Apply the PANS/PANDAS diagnostic criteria to a child presenting with abrupt-onset OCD, restricted eating, tics, or behavioral regression, and formulate an initial infectious, inflammatory, and immunologic workup that distinguishes PANS from autoimmune encephalitis and primary psychiatric illness.
• Assess flare severity and trajectory in a patient with a post-infectious neuropsychiatric syndrome and recommend a severity-matched treatment plan that integrates psychotherapy, low-dose psychotropic prescribing, antimicrobial therapy, and escalating immunomodulation (NSAIDs, corticosteroids, IVIG, second-line agents).
• Evaluate the current evidence for immune-targeted treatment in PANS/PANDAS and analyze how a multidisciplinary, research-integrated clinic model such as the Dartmouth NIPD program can be adapted to their own institution to reduce diagnostic delay and support prospective study.
