Overview and Target Audience:
By participating in this discussion, the learner will identify systems that can be improved and spark process improvement initiatives across the system. We are working on building a multidisciplinary team culture where we work together to address opportunities for better patient care within an optimized and resilient work environment.
Global Objectives:
During each session, faculty and participants will:
- Perform objective nonjudgmental review of (adverse) outcomes.
- Identify systems' conditions that contribute to medical errors and/or patient harm.
- Determine systems' process interventions that are safe, timely, effective, efficient, equitable, and patient-centered.
Learner Outcome:
At the conclusion of this monthly multidisciplinary case review, participants will be able to analyze factors contributing to medication errors or patient harm within a clinical case and identify at least one evidence-based strategy they can implement in their own practice to enhance safe and effective patient care.
Continuing Education Credit:
Registration, attendance, and evaluation are required for successful completion to obtain professional continuing education credit.
